What this page is. A publicly searchable index of the medical professionals named as subjects of Daniel Perry's Form 2 Information under s.504 of the Criminal Code of Canada. The Information lays 29 counts against Capt. Rebecca Covey (CFNIS) and others; the "others" include named medical professionals alleged to have participated in the pleaded misconduct.

What this page is NOT. A conviction record. A finding of fact. A substitute for court proceedings or regulatory investigation. None of the persons named below have been found guilty of any criminal offence. None of the persons named have been subject to a regulatory finding of professional misconduct on the specific allegations pleaded. The pleaded allegations remain untested.

Why this page exists anyway. The s.504 Information is filed. It is a public document. The allegations in a filed Information are, by the operation of s.504 itself, part of the public record. TENET5 is a journalism-in-public investigation; the function of this page is to index the names and the specific counts alleged so that the Information is searchable by anyone reviewing it — including the College of Physicians and Surgeons of Ontario, MPCC, CFNIS, and any future court.

Correction policy. Any named person who believes a statement here is inaccurate or unfairly characterised beyond what the s.504 Information itself alleges may request a correction. TENET5 will update the page within 48 hours of verified notice from the named person's counsel. Contact via the About page.
